REQUIREMENTS FOR ALL ATHLETES

Each student athlete is required to set up a DragonFly account prior to participation.

DragonFly requires the following forms to be uploaded:

• Birth Certificate
• Physical
• Insurance Card

In addition, ALL student athletes are required to fill out the following forms and return them to thier coach BEFORE participating in any practice or game. 

• Physical (must be filled out by physician & completed fully)
• Concussion Form
